Matrix Medical Network is a physician practice developed to provide high-quality, cost effective care to patients in long-term care settings.

We employ physicians and nurse practitioners who are focused solely on the care of patients in nursing homes, assisted living facilities and other long-term care facilities. Members of our care team often do not have office based practices; their priority is delivering the best possible care to patients by working with the facility, its administration and staff.

We partner nationally with managed care organizations and long-term care facilities to improve the health status of long-term and short-term care patients.
