- Matrix's mission is to provide high quality primary care physician services to residents of nursing homes, skilled nursing facilities, and assisted living facilities.
- The Matrix model requires that the physician focus on these settings, rather than on an office or hospital practice. We believe that this model enhances the quality of the patient-physician relationship and results in better outcomes.
- Since most Matrix physicians do not provide services in the hospital or office setting on a regular basis, they do not refer patients from these settings to nursing homes, skilled nursing facilities, or assisted living facilities.
- While Matrix does contract with several health plans to care for their members in nursing homes and skilled nursing facilities, Matrix does not control the decision as to where a health plan member receives care. That decision is made by the health plan and the member.
- Regardless of any protocol that may be utilized by a facility for matching patients to physicians (such as assignment by floor or a "rotation"), Matrix recognizes that the final determinant in the selection of a physician by a patient is patient choice. Matrix does not require that any patient in any facility use our physician.
